Jan Tomáš Forman, known as Miloš Forman, was a Czech and American film director, screenwriter, actor and professor, born in Čáslav, Czechoslovakia, on 18 February 1932. He died on 13 April 2018. He rose to fame in his native Czechoslovakia before emigrating to the United States in 1968, and over a career of six decades, from 1953 to 2011, he won two Academy Awards, a BAFTA Award, three Golden Globe Awards, a Golden Bear, a César Award and the Czech Lion. Within this atlas Milos Forman is credited as the director of One Flew Over the Cuckoo's Nest and Amadeus (1984 film).
